Directions for on-line Youth Sports registration:

Find the sport(s) for which you wish to participate, from the choices below.

The parent information letter for the sport(s) you choose will print out with the form. You are responsible for the information contained in the letter. You would be wise to keep the letter until seasons end.

Complete the form. Any missing information will cause the form to be returned to you and will cause a delay in your child’s registration process.

Please make sure you submit the right amount of money.

The map below shows the City of Coopersville boundaries. If you live outside of this boundary, you are a non-city resident.

Costs are:

Costs City of Coopersville Residents $40.00 per child
Non-residents of the city $55.00 per child per child
GLSL U-10 and up costs:
Jersey $20.00

Copy of birth certificates required for all first year players. Players must be registered by parents or legal guardian.

If the correct fees are not received, registrations will be returned and children will be placed on a waiting list. They will be placed on a team IF there is room.  We have maximized our capacity and will have to turn away kids when we reach maximum numbers.

Make sure the shirt size is correct. What you order is what you will receive.

All requests must be written on the form. We will not change rosters after the coaches meeting.  Requests are not guaranteed.

Complete the form include the appropriate fee and copy of birth certificate if necessary and mail toFall Sports 289 Danforth Coopersville 49404. Also, you may place the form in the drop box located in front of city hall.

You will be notified by a coach as to practice times and locations.
